Knights of Columbus Free Throw Championship
Sunday January 24, 2010 - St. Anthony School Gym
Registration 2:00 PM, contest shortly after
Contestant's age determined as of January 1, 2010.
Copy of birth certificate required.
General Information:
There will be 5 boy and 5 girl winners.
Contestants ages 10 and 11 will shoot from 12 feet instead of the normal 15 feet.
Contestants will have 3 warm-ups and then 15 free throws.
Tie breaker; no warm-up and 5 free throws.
Winner plaques awarded at the event; name plates later.
Winners will participate in the District event in February, and if a winner were not available for the District event the runner-up would take their place.
District winners will participate in the State event in March.

Parents please read the form completely.
Complete the entry form and attach a copy of the birth certificate.
The contestant needs to sign the form, and the parent needs to sign the form which includes a release.
Please note that high school participants need to check with the school athletic director about eligibility before participating.
Bring the form and birth certificate to the event or you can pre- register by mailing the entry form with a copy of the birth certificate to the address below.
